If you were enrolled in the Limited Purpose and/or Dependent Care Flexible Spending Account for the July 1, 2016 to June 30, 2017 plan year, you have until September 30, 2017 to submit claims to Payflex for reimbursement. These claims would be for expenses incurred while you were active in the plan during the 2016-2017 plan year. Have your receipts ready, as you may need to submit with any claims in order to verify the expense(s) being requested for reimbursement.
Claims should be filed promptly, as any balances remaining in your account(s) for the 2016-2017 plan year will be forfeited after September 30, 2017. Claims can be submitted on HealthHub, HealthHub® - Login or mailed using the claim forms that can be found here.
