If you incurred eligible dental or vision care and/or dependent care expenses between July 1, 2016 and June 30, 2017, you must submit your claims to Payflex, our FSA administrator, by September 30, 2017 to receive reimbursement.

If you terminate employment, no longer participate or become ineligible for the FSA prior to June 30, 2017, all eligible expenses incurred through the date your employment ended, the date you no longer participated or the date you are no longer eligible, must be submitted by September 30th following the end of the current plan year.
