If a you're enrolled in a Toys”R”Us medical plan, you and your covered spouse/domestic partner can earn up to a $700 deposit into your Health Savings Account (HSA) by participating in the 2015-2016 Wellness Rebate and Wellness Incentive Programs!** 

If you and/or your covered spouse/domestic partner haven’t done so already:

  • Qualify for the Wellness Rebate by completing a biometric screening and online health assessment through guidanceresources.com to earn up to a $500 HSA deposit* ($250 if the team member completes; $250 if your covered spouse/domestic partner completes.)
    • First-time users enter Company ID: EAP4TRU; Spouses/domestic partners must create their own profile.
  • Qualify for Wellness Incentives by completing one or all of the following activities to earn up to a $200 HSA deposit* (up to $100 if the team member completes; up to $100 if your covered spouse/domestic partner completes.)
    • Go to guidancereources.com to complete an Online Coaching Program > 40 points = $40 HSA deposit
      • Online Coaching Program takes between 3-6 weeks to complete.
    • Call 1-866-519-8360 to complete a Telephonic Coaching Program > 40 points = $40 HSA deposit
      • A program is 5 calls
    • Complete a Healthy Activity and self-report on guidanceresources.com > 20 points = $20 HSA deposit
      • Examples include participating in a 5K, blood drive, gym membership, etc.

The final deadline to complete and submit the 2015-2016 Wellness Rebate and Wellness Incentive Program activities is April 30, 2016.   .
